Alternative Septic Systems
The following MUST be completed before issuing the Certificate of Compliance.
- Notice of Alternative Sewerage Disposal System (2-page document) (Filled out by Engineer; signed, notarized and recorded by homeowner at Plymouth Registry of Deeds or Lands Court)
Notice of Alternative Sewerage Form
- Alternative Technology Certification by Owner -filled out and signed by owner (1 page document that does not need to be recorded)
Alternative Technology Certification Form
- A copy of Maintenance & Monitoring Agreement (provided by homeowner)
- Stamped Certified As-Built Plan and Stamped Certificate of Construction from Engineer
- Septic Installer sign off (submitted by septic installer)
